Sign Up for an Educational SessionMany patients begin by taking one of our bariatric surgery informational seminars.
If you would prefer, you also may begin by contacting our patient coordinator, Eileen, by calling 303-789-1877 ext. 101 or toll-free at 1-800-332-7291. She will help you set up your first appointment with the Colorado Bariatric Surgery practice and also help ensure that you are an appropriate candidate for the procedure.
Your First AppointmentPrior to your first appointment, an informational packet will be sent to you. Please bring a completed medical history form to your first appointment. Or you can have one mailed or faxed to you by asking Eileen during your initial call.
